WHO can get the vaccine: Milton-Union students, parents, families, staff and staff families.
WHEN is the clinic: Tuesday, October 19, from 1:30 p.m. to 4:30 p.m.
WHERE is the clinic: Milton-Union Middle School (just follow the signs inside).
HOW MUCH will the vaccine cost:
Kids 18 & under – $10 for shot or nasal
Adults 19 & up – $20 for shot, $30 for nasal
They will accept cash, check, credit card (Mastercard and Visa).
Insurance accepted for kids – Medicaid, CareSource, AmeriGroup & Molina.
Insurance accepted for adults – Medicare
Limited supply of vaccine for students 18 years old and younger. Vaccine will be given in the order forms are received.
WHAT do you need to do:
Kids 6 months – 4th grade: Print and fill out the form found at http://www.milton-union.k12.oh.us/Lists/Announcements/Attachments/101/MiltonUnionFluClinic2010.pdf, Return the form and payment to school no later than Thursday, October 14th (there is no school on Friday, October 15th), Come to the clinic after school with your children for vaccination (parent or guardian present is required for this age group.
Students 5th grade – 18 years old: Print and fill out the form found at the link posted above, Return the form and payment to school no later than Thursday, October 14th (there is no school on Friday, October 15th), Your child will be vaccinated during the school day – you don’t need to do anything else. NOTE: If you want to be present during vaccination, note this on the form and come to the clinic after school.
Adults age 19 & older: Print and complete the form found at http://www.miamicountyhealth.net/PHN/documents/AdultFluRegForm.pdf, Return the form and payment to school no later than Thursday, October 14th (there is no school on Friday, October 15th).
Flu cases are already starting to appear in the Miami Valley. The seasonal flu vaccine will give you protection for 6-8 months.
