SpringMeade Health Center earns 3rd Deficiency Free Survey SpringMeade Health Center, a Premier Health Skilled Nursing and Rehabilitation Facility, earned their third consecutive Ohio Department of Health deficiency free survey in February 2016. One of the highest validations of quality in Skilled Nursing Facilities is to receive a deficiency free survey from the Ohio Department of Health. Each nursing Facility in Ohio receives at least one unannounced inspection/survey during a 9 to 15 month inspection cycle. During these surveys all aspects of care and services are evaluated based on state and federal laws and rules.
